Case IH 19746 – Engine Control Unit (ECU): Maximum negative rail pressure deviation with closed pressure control valve exceeded (second stage)
Issue description
This fault code indicates a severe under-pressure condition in the high-pressure common rail fuel system. The Engine Control Unit (ECU) has detected that the actual fuel rail pressure is significantly lower than the target (desired) rail pressure. The system has attempted to correct this by commanding the fuel pressure control valve (or fuel metering unit) to fully close (its maximum limit) to build pressure, but the pressure still fails to reach the required threshold. The "second stage" designation means the deviation is severe or has exceeded a prolonged time limit, which will typically result in an engine derate, hard starting, or the engine shutting down entirely to prevent damage.

Check the cause
Low-Pressure Fuel Supply Check: Verify that the low-pressure lift pump is supplying adequate fuel to the high-pressure injection pump. Check for clogged fuel filters, gelled fuel, or air leaks in the low-pressure lines.
Injector Back-Leak (Return Flow) Test: Connect graduated tubes or digital flow sensors to each injector's return fitting. Compare the volume of fuel returning to the tank while cranking or at idle. Excessive back-leakage indicates worn internal seals or a failing injector that is bleeding off the rail pressure.
Pressure Control Valve Test: Evaluate whether the fuel pressure control solenoid valve is causing the low pressure by allowing excessive fuel to overflow into the return line.
Rail Pressure Sensor Verification: Monitor the rail pressure sensor voltage during cranking (it should typically read over 3600 PSI or 1.0 volt to allow starting). If the sensor is misreporting, the ECU will not inject fuel.
Visual Inspection: Check the high-pressure lines, rail, and pump for any external fuel leaks.
Repair steps
Replace Fuel Filters
Start by replacing the primary and secondary fuel filters and bleeding the low-pressure fuel system to rule out a simple restriction.
Address Leaking Injectors
If the back-leak test reveals uneven or excessive flow across specific cylinders, remove and replace the faulty fuel injector(s).
Replace the Pressure Control Valve
If the injectors pass the return flow test but rail pressure cannot be maintained, replace the fuel pressure control valve (solenoid) located on the high-pressure pump or fuel rail, as it may be mechanically stuck open or electrically defective.
Replace the Rail Pressure Sensor
If diagnostic scanner data shows implausible pressure readings compared to a manual gauge, replace the rail pressure sensor.
Replace the High-Pressure Fuel Pump (CP Pump)
If the low-pressure supply is good, injectors are not leaking, and the control valve is functioning, the high-pressure injection pump has likely suffered an internal mechanical failure and must be replaced.
Clear Codes and Test
Use a diagnostic scanner to clear the fault codes. Run the engine under load to verify that actual rail pressure matches the commanded setpoint without deviation.
