Issue description
This code indicates an electrical failure in the output driver circuit that controls the #3 rear electro-hydraulic remote (EHR) valve. The system's controller is detecting that the electrical signal/power being sent to the solenoid for the #3 remote spool is compromised, which usually points to an open circuit, a short to ground, a short to power, or an internal failure of the controller's output stage.

Check the cause
Inspect the wiring harness and connectors between the EHR controller and the Rear Remote No. 3 valve block for pinched wires, rubbed-through insulation, or corroded connector pins.
Use a multimeter to check the No. 3 remote harness for continuity, short-to-ground, and short-to-power.
Measure the resistance of the No. 3 remote valve solenoid coil to ensure it is not internally shorted or open.
Swap the electrical connector from Remote No. 3 to a known good remote (like No. 2 or No. 4). If the fault code changes to the new remote (e.g., NH 4142/4152), the issue is the valve solenoid. If the code remains NH 4138, the issue is in the wiring harness or the EHR controller itself.
Repair steps
Power Down
Turn off the tractor and disconnect the battery to prevent electrical spikes during repair.
Resolve Wiring Issues
If the diagnostic checks revealed a broken or shorted wire, repair the damaged section of the harness or replace the remote valve harness pigtail.
Replace the Solenoid/Valve
If the swap test proved the solenoid is faulty, remove the retaining nut on the No. 3 remote valve block and replace the solenoid coil/cartridge.
Replace the Controller (Worst Case)
If the wiring and solenoid both test perfectly, the 'Output Stage Faulty' designation means the internal driver inside the EHR controller has burned out. The controller will need to be replaced.
Calibration
If a CAN-based EHR valve or controller is replaced, you must assign the correct valve address (telling the system it is valve #3) and perform a spool travel calibration via the tractor's in-cab display before it will operate correctly.
